Access your client portal today!

Our practice uses a HIPAA compliant web portal called OnPatient that allows clients to access a portal to view their records, complete and update their health history and contact information, and electronically sign consents and read educational forms.  

To sign up for the first time, you will need a Client Portal Sign-Up Link. This will be emailed to you by our office if you have not received a link please call or text the office and let us know at 229-234-2505 (text/call) or email us at adminassist@midwifeme.org and someone will reset your access. 

With the client portal you can:

• Send messages to your healthcare providers, office staff, or billing team
• Access and sign consent forms
• Review educational forms and handouts
• Fill out your health history
• Login to complete your telehealth appointments
• View upcoming scheduled and previous appointment times / dates
